Please read below to ensure you complete the registration process correctly.
Upon completing the registration form you will be taken to your account page where you must click the Subscribe Now button to set up your membership payment of $28.95/mo, or fill out the ACH Authorization Form and fax it to 937-660-5659.
Within 48 hours of setting up your membership payment you will receive an email containing you Teladoc ID number needed when calling into Teladoc to schedule a consultation call with a doctor. Once you receive this email your registration has been completed and you are now in the Teladoc system.
